      I have read the instructions listed separately, and I wish to receive the official application for a mail-forwarding service in Fairbanks, Alaska.  I understand that the yearly fee for forwarding up to approximately 12 letters per year (regardless of how many names are used) is $150*, and that if many additional letters are received, my annual rate may be raised for the following year.*  

* Option: For an annual fee to Alaska of $200 instead of $150, you can receive approximately 30 letters per year. (If you choose this option, please circle the "$200.")
